Pets of the Year 2009 Contest Form--------------------

Enter ypur pet's photo for a chance to be one of the 12 finalists for out Shelby Humane Society Inc. Pets of the year 2009 Calendar.

Simply make a $10 donation to the Shelby Humane Society Inc. and send your pet's photo to 1309 W. S. 7th, Shelbyville. All photos will be on display in our building for judging by the local community.

The top 12 winners will be awarded a private photography session for your pet and family, a pet gift basket with goodies for your pets and coupons for you.
